Foreign National Tax Status Review Appointments 

As a precautionary adjustment due to the COVID-19 outbreak, all in-person tax status review sessions have been suspended. Tax status reviews will be conducted electronically, and registration is still required. Further instructions regarding submission of the required documentation will be provided via email once registration has been confirmed.

Purpose of Tax Status Review Appointments

The Foreign National Service Center (FNSC), in University Payroll & Benefits (UPB), offers tax status review appointments for foreign national employees to provide a more efficient means of processing tax forms and potential tax treaty benefits. FNSC offers mass appointments in Urbana-Champaign and Chicago, and individual appointments in Springfield. Per IRS regulations, employees’ federal taxes will be withheld at the highest possible rate until after they have completed the tax status review appointment.

UPB does NOT provide tax advice and does NOT assist with income tax returns at any time.

Requirements (Read ALL information below.) 

  • Submit copies of the required documentation (see #1 and #2 below) as requested in your registration confirmation email. Your appointment will not be completed until all required documentation is received.

How to Complete a Foreign National Tax Status Review Appointment

  1. Complete Foreign National Tax Information Form either electronically, or with a blue or black ink pen.
  2. Submit copies of the required documentation as requested in your registration confirmation email based on your previous and current visa types.
  • Social Security Number (SSN) card, Individual Taxpayer Identification Number (ITIN), or Receipt of Application  
  • Passport ID page
  • Passport renewal page
  • All U.S. entry visas
  • All U.S. entry stamps
  • I-94 record (electronic, or front and back of card)
  • Offer letter, or Notice of Appointment (if available)
  • F-1 Visas: All I-20s (all pages)
  • F-1 Visas: Employment Authorization Document (EAD) or Optional Practical Training (if applicable)
  • J-1 Visas: All DS-2019s
  • H-1B Visas: I-797 (original not required)
  • Any documentation for change of status

 NOTE: Generally, new appointment dates are posted on the 2nd Friday of each month.
