  • Page one of this form must be completed by all alternate drivers of a University motor vehicle. An alternate driver is any driver other than the driver whose name and information is contained on page one of the Car Release Order that was used by the department to obtain the use of the motor vehicle. An Alternate Drivers Car Release for each alternate driver should be attached to the Car Release Order when checking out a University motor vehicle.
